Privacy Policy


The FallsAssistant application can be accessed via the FallsAssistant Domain. This domain is managed by The Social Marketing Gateway on behalf of NHS24:

  • NHS 24, whose principal place of business is SmartCare Programme Office, Caledonia House, 140 Fifty Pitches Road, Cardonald Park, Glasgow, G51 4EB (Data Protection Registration Number: Z8707451)
  • The Social Marketing Gateway, whose principal place of business is Foxter Chambers, 28 Bath Street, Glasgow, G2 1HG (Data Protection Registration Number: Z2416343)

For the purposes of the Data Protection Act 1998, NHS 24 and The Social Marketing Gateway ‘We’ are operating as Data Controllers in common, meaning that they are responsible for the way in which your data is collected, stored and processed.

As part of the programme, NHS 24 and The Social Marketing Gateway may provide data collected through the operation of the FallsAssistant Domain to any NHS Health Board or Trust, in order to allow that Health Board or Trust to provide its services to you. In those circumstances the Health Board or Trust concerned will also be operating as a Data Controller, and will process any personal data in accordance with its own privacy policy.

This Privacy Policy, together with our Terms and Conditions and Cookies Policy, sets out our data processing practices, including how and for what purposes we use the information collected about you.

Information Collected

We collect personal information, including sensitive personal information, that you submit and through your use of the FallsAssistant Domain. We will use this information for the purpose of improving the quality of the services we provide to you.


We will collect the following information that you submit via the LIU Domain:

  • Your name and contact information (such as postal address, location (region), email address, and mobile or other telephone number);
  • Username and password;
  • Any information that you enter in your action plan


We will collect the following information automatically from your computer:

  • Information from cookies (see below for a fuller explanation of cookies and their function).
  • Your IP address. This is the unique address of your computer and is automatically provided to other computers when your web browser or email application requests a web page or email from those computers on the Internet.
  • Information about the type of browser you are using (for example Firefox, Internet Explorer or Safari).
  • The time and date on which you created your FallsAssistant Domain account and the times and dates of your subsequent visits.


Use of personal information

We shall use any personal information we collect in accordance with this Privacy Policy. Save as expressly allowed under this Privacy Policy or required by law, we shall not disclose your personal information to any third parties without obtaining your prior consent, unless required to do so by law.


We may use the information described above to:

  • Create and manage your profile;
  • Provide products and services;
  • Deliver patient support;
  • Send promotional materials and other communications;
  • Communicate with you about and administer your participation in special events, programmes, offers, surveys and market research;
  • Respond to your enquiries;
  • Operate, evaluate and improve our services and products;
  • Perform data analyses for strategic development purposes to measure the effectiveness of content/ideas and to provide you with a more personalised service; and
  • Create anonymised data which we may then disclose to organisations that support the LIU Programme, allowing us to improve and extend the services and products made available through the LIU Domain.


Information we share

We do not disclose the personal information we collect about you, except as described in this Privacy Policy or as required by law. We may share information relating to you with your medical professionals, whose use, disclosure and other handling of your personal information will be governed by their own privacy policies and practices.


We may share your personal information with our affiliates for the purposes described in this Privacy Policy. We may also share your personal information with service providers who perform services on our behalf based on our instructions. We do not authorise these service providers to use or disclose personal information except as necessary to perform services on our behalf or to comply with legal requirements. We impose strict legal obligations on these service providers requiring them to implement robust security measures designed to protect your personal information.


We store the personal information we collect on computer systems with limited rights of access; located in controlled facilities. When we transmit highly confidential information (such as a password) over the Internet, we protect it through the use of encryption technology such as the Secure Socket Layer (SSL) protocol.


If a password is given to you to help protect your accounts and personal information, it is your responsibility to keep that password confidential. Do not share that information with anyone. If you are sharing a computer with anyone, you should always log out before leaving a site or service to protect your information from being accessed by other users.


Can I remove my information?

Of course, we believe that you should be in control of your data and should be able to delete it at any time. If you are a registered user of FallsAssistant you can delete your profile at any time emailing support@livingitup.org.uk and requesting that your account be deleted.


You also may at any time request access to all personal information held on you by NHS 24 by email - dp@nhs24.scot.nhs.uk or by writing to SmartCare Programme Office, Caledonia House, 140 Fifty Pitches Road, Cardonald Park, Glasgow, G51 4EB. Further, if your personal details change or if you have any queries about how we use your information, please let us know by contacting us as set out above.

The FallsAssistant Domain contains links to other sites, including those of our third party partners. Linked sites and applications may have their own terms and conditions, which we strongly suggest you read. We are not responsible for the privacy practices of the third parties operating those sites. In particular, unless expressly stated, we are not agents for the operators of those sites nor are we authorised to make representations on their behalf.


Safeguards, security and storage

Although we do our best to protect your personal data, we cannot guarantee the security of the data transmitted to the FallsAssistant Domain or any associated SmartCare applications. Any transmission is made at your own risk. However, we will endeavour to protect all personal information collected through the FallsAssistant Domain in accordance with strict data protection standards.


All of the information we collect or record is restricted to our regional officers or designated secure hosting. Only employees who need the information to perform a specific job are granted access to personally identifiable information.


We will retain your information for as long as we are providing you with services. We will also retain your information for a longer period if required to by law.
